Common Destiny , a residential home for seniors located near Northgate Park in Fremont offers an alternative to home care in a quiet family oriented neighborhood. We have been in business since 2010.
We provide all the standard care services for the elderly such as assistance with daily living, medication and medical appointment management, and compassion. Our caregivers provide 24 hour loving care. We can provide ambulatory and non-ambulatory service as well as hospice care.
Basic services provided:
– Care and supervision
– Observation for changes in physical, mental, emotional, and social functioning
– Lodging: One private single room- Food services: to include 3 nutritious meals daily and snacks, special diets if prescribed by a doctor
– Assistance to gain access to supportive services, such as voting, senior assistance agencies, etc.
– Assistance in making arrangements for transportation with a 3rd party provider as requested
– Assistance with personal activities of daily living
Additional basic services included in the Basic Services fee:
– Hygiene items of general use, such as soap and toilet paper
– Laundering of personal clothing
– Clean bed and bath linens weekly, or as often as needed
– Cleaning of resident’s room
– Comfortable and suitable bed and/or bedroom furniture
– Assistance in meeting necessary medical and dental needs
– Assistance with taking prescribed and over-the-counter medications in accordance with physician’s instructions unless prohibited by law or regulations.
– Bedside care and tray service for minor temporary illnesses or recovery from surgery.
